The Social Prescribing Coordinator is a fixed term full-time contract for a three month period (with the possibility of a further extension subject to funding). It is an exciting and challenging opportunity for an enthusiastic motivated individual to become part of the vibrant team at Sacred Heart Family Resource Centre where you will be based. Outreach work will be a key feature of the role.
Social Prescribing is a structured programme of non-medical supports aimed at improving and protecting an individual’s health and wellbeing. The project will create links between medical practitioners and community based organisations to encourage individuals to access a wide range of non-medical activities to enhance their personal health & wellbeing.
As Social Prescribing Coordinator you must have experience of at least 3 years in a Community, Youth and/or Health setting of working on the wider determinants of health, in identifying services and activities and supporting people to access same.
This project will focus on the Waterford Metropolitan Area developing an innovative approach to health and wellbeing.
The position will necessitate the following:-
- The successful candidate must have a recognised level 7 Degree or higher in Social, Community, Health or other related field and a minimum of 3 years experience in a relevant setting.
- A full driving licence and access to a car for work purposes.
- Experience of project administration and co-ordination.
